37 hours per week  

£25,069.72 per annum

An exciting opportunity has arisen for a self-motivated, proactive, and efficient individual to join Healthworks on our Community Wellbeing Pathfinder service to promote and encourage healthy lifestyles and play a vital role in supporting individuals and communities to improve their overall wellbeing. 

About Us 

Healthworks is a national award winning charity working with disadvantaged local communities to tackle health inequalities and improve their health and wellbeing through social prescribing. Based in Newcastle, but working across the region for over 30 years, our patron is Professor Michael Marmot. Our work has been recognised by a King’s Fund/GSK award for our work improving the physical and mental health of people across the North East of England.   

We work in partnership with NHS and Local Authority colleagues and communities in many sectors to devise cost effective solutions that secure sustainable health impacts for many conditions that inhibit quality of life. 

The Role 

You will provide brief advice, short and longer interventions, and signposting or referrals to help clients access the support they need. 

You will;  

  • Deliver one-to-one wellbeing support to individuals, building trusted and supportive relationships  

  • Co-produce personalised wellbeing plans based on individuals strengths, needs, and priorities  

  • Support individuals to identify and overcome barriers to improving their wellbeing  

  • Deliver support flexibly at Healthworks centres, in community venues, through outreach locations, or by phone  

  • Support individuals to access and engage with local services, community groups, and statutory agencies  

  • Maintain strong local knowledge of community resources to effectively signpost and support individuals  

  • Maintain a visible community presence through outreach and drop-in sessions
