The North East and North Cumbria Ageing Well sub-group, part of the VCSE Partnership Programme, works alongside the Integrated Care System (ICS) Ageing Well Workstream that is shaping the services and support for older people across the region.
A Frailty Outcomes Framework (FOF), which utilises NHS service data, has been developed for and by the NHS to help plan service design, feed into decision making and to support local areas to see where improvement lies. They are looking at metrics such as frailty assessments and falls, flu, dementia, reablement, quality of life and care homes.
The VCSE Ageing Well sub-group is creating a supporting document for the ICS’s Frailty Outcomes Framework (FOF) report. This aims to showcase innovative VCSE sector services in order to highlight to commissioners and health partners what can be achieved by our sector that supports their outcomes.
We’re looking for examples of work to be included. So if you support and provide services for older people in the region, please do share information about your projects and services with us through this short online form.
