Edberts House is a thriving community charity currently managing four community projects based in Gateshead. We work together with residents of all ages to make our communities happier, healthier, and more welcoming places to live. 

Over the past six years our Community Linking Project has contributed to the health and wellbeing of patients in Gateshead by linking them to a range of community services. We have a vacancy for a Community Link Worker to join our social prescribing team. You will be employed by Edberts House to work within the Outer West Gateshead  Primary Care Networks (PCN) to support patients who have visited the GP by providing advocacy, signposting, and accompanying individuals to local, non-medical sources of help. You will proactively build relationships with patients and community organisations to support individuals to come up with innovative solutions to problems. 

Edberts House provides:

  • In depth induction programme including work shadowing (within covid guidance)
  • Access to role specific training and development opportunities
  • Regular support and supervision from designated Operational Lead
  • Caseload management and reviews
  • Service wide team meetings and peer support with other CLWs
  • Support with community mapping and engagement with partners
  • Involvement and participation in new projects and other SP related developments

Candidates will come from a community, information/advice, social care, or health background, must be excellent communicators and able to work in a multi-disciplinary team.  You will need an interest in working with people with multiple and complex needs, and experience of supporting people with long term conditions would be an asset.

An enhanced DBS is required for this post as well as proof that you are fully vaccinated against Covid -19 (or have a approved and agreed medial exemption)
